Step 1: Schedule Your Exam
Please contact our office at 979-541-2739 to schedule an initial evaluation. This will give us the information we need to determine how much vision therapy is needed so that we can provide you with an accurate quote for therapy. Our staff will ask you some preliminary questions about your or your child’s symptoms to determine if both a visual acquisitions skills and perceptual evaluation are needed or if just one is appropriate. Step 2: Complete Your Medical History
You have two options. You can click on button to the right and fill out the form electronically, or you can download and return the completed registration form below.
We must receive your completed registration form to our office by the Friday before your appointment. Our fax number is 979-541-2739. If you do not have access to a fax machine, you can drop the form off at our office. You will need AdobeReader® to download and complete the forms. Choose from one of the below registration forms:
Step 3: Request Medical Records from Your Eye Doctor
The last step is to request that your primary eye doctor fax a copy of your medical records to office. This way, we have a full picture of what is happening with your eyes. Please download, sign, and return the below form to us ASAP. We’ll contact your provider directly and ask them to provide us with records before your evaluation.
